There were several ineffective aspects of how the SM managed the contractor while on site at the café, including:

1. Failure to return the owner's call and acknowledge the need for supervision of the contractor on the day of the repair.

2. Poor communication with the workers and the contractor regarding the repair work, leading to confusion and mismanagement.

3. Neglecting to inform the workers of the contractor's arrival and purpose, leading to the worker at the counter being unprepared and not knowing what to do.

4. Refusing to supervise the contractor and delegating this responsibility to a worker who was not adequately informed or trained to do so.

5. Failing to ensure the contractor had the necessary information and tools to complete the repair safely and efficiently.

6. Prioritizing the turnover of customers over the safety and wellbeing of the workers and customers.

7. Failing to assess and manage the risks associated with bringing additional furniture inside during a busy and rainy day.

8. Insufficient training and support provided to the workers to manage increased workload and customer demands.

9. Ignoring concerns raised by workers about the increased workload and its impact on their health and Well being.

10. Inadequate management of worker sick days, leading to increased absenteeism and potential loss of productivity.

11. Lack of attention to workplace hazards, such as the step leading to the second seating area, which could cause accidents and injuries.

Answer.
Comments on the effectiveness of role and responsibilities of Café owner:

  1. The owner of the cafĂ© shows leadership and commitment toward health and safety in a very effective manner
  2. Hiring a Shift manager for Additional working days and manage overload by hiring temporary staff
  3. when he was informed by a worker about faulty of coffee machine, he inspects it and direct to all worker to stop using it also he put “out of order” sign on it
  4. He communicates the message to the shift manager regarding faulting of machine
  5. He also hires a contractor for maintenance of faulty machine
  6. Previous Record of near misses and accident book are maintained by cafĂ© owner
  7. He encourages a proper way of reporting hazard and near misses when I informed him regarding “tripped hazard he immediately hung a ‘mind the step’ sign on the wall next to the step.
  8. He also takes care of the injure worker.
  9. He was ready and plan to do investigation on the accident

Comments on the ineffectiveness of role and responsibilities of Shift supervisor:

  1. In the scenario the accident occurred because the role and responsibilities of the Shift Manager is ineffective,
  2. SM is irresponsible and not performing his responsibility effectively, he Pretend to be busy every time
  3. The SM did not develop and implement appropriate training programmed for any of the worker.
  4. Risk assessment not carried out before
  5. The SM is not providing proper supervision, he is aggressive, not supportive, no worker consultation carried out as he is only concern about the profitability rather than the well being of their worker
  6. SM not encourage the workers to maintain safe working environment like reporting incident, injuries and near misses, when the accident happened then I tell the SM where the accident book is kept, as the owner would usually complete this. The SM replied that they will complete it later and force me to focus on serving customers.
  7. SM not define balance workload, effective welfare and incentive programmes to boost employee engagement.
  8. SM refused to supervised the contractor for maintenance work, and allow untrained worker to supervise the contractor
  9. Preventive Maintenance of all the equipment’s not performed by SM
  10. No Proper Reporting and Communication hierarchy developed by SM
  11. SM is not willing to conduct and initiate incident Investigation
SM not aware about how to deal with the emergency situation

Comments on the ineffectiveness of role and responsibilities of Full and part time workers:

  1. The role and responsibilities of all the worker is also ineffective as:
  2. The workers are incompetent and not serious about the health and safety they not taking reasonable care of their own safety as well as the safety their colleague.
  3. Fail to manage the contractor effectively
  4. No one refuse to work in an unsafe environment
  5. SM exerted a great deal of pressure on the workers, and as a result, everyone was working as quickly as possible. so that they are unable to care about health and safety
  6. Not taking part in assisting incident and accident investigation

Comments on the effectiveness of My role and Responsibilities:

When accident happen, I was the one which responded to the injured worker in a very effective manner by providing first aid to the injured worker, taking permission from SM to report in safety book, communicate to the café owner about the accident, and willing to assist in accident investigation.
